If you’ve lost most of your teeth – whether from decay, gum disease, or injury – you’ve probably been told your main options are dentures or replacing every single tooth with its own individual implant. Both come with real downsides: dentures can shift, cause irritation, and never quite restore full chewing power, while a full mouth of individual implants can mean months of surgery and a hefty bill. All-on-4 sits in between the two, and for a lot of patients, it’s the better answer.

What Is All-on-4?

All-on-4 is a technique for replacing an entire arch of teeth – the full upper row, the full lower row, or both – using just four implants per arch, instead of one implant for every missing tooth.

Two of the implants are placed upright towards the front of the jaw. The other two are angled, roughly 30–45 degrees, towards the back. That angling isn’t just for stability: it’s a deliberate strategy to anchor the implants in the denser bone that’s usually still available towards the back of the jaw, even in patients who’ve experienced some bone loss. This is a big part of why All-on-4 often avoids the need for bone grafting, a step that’s frequently required with other implant approaches.

Once the four implants are in place, a full, fixed bridge of replacement teeth is attached on top. In many cases, this can be done on the very same day as your surgery. That means walking in with missing or failing teeth and walking out with a functional, natural-looking smile.

Why All-on-4 Rather Than Dentures Or Individual Implants?

Compared to dentures:

  • All-on-4 is fixed in place: no slipping, clicking, or need to take it out at night
  • Chewing strength is much closer to that of natural teeth, so eating a wider range of foods is realistic again
  • There’s no denture adhesive, no soaking overnight, and no worrying about it during conversation

Compared to individual implants for every tooth:

  • Far fewer implants are needed to restore a full arch, which typically means a shorter procedure and a lower overall cost
  • The angled rear implants make use of existing bone, often removing the need for bone grafting
  • Because it’s one connected bridge rather than many separate implants, the process is generally quicker from start to finish

Are You A Candidate?

All-on-4 tends to be a strong fit if you:

  • Have lost most or all of your teeth in one or both jaws
  • Currently wear removable dentures and are unhappy with how they fit, feel, or function
  • Have been told in the past that you don’t have enough bone for implants
  • Want a permanent, fixed solution rather than something removable

Bone loss, in particular, is one of the most common reasons patients assume they aren’t candidates for implants, but All-on-4’s angled placement is specifically designed to work with the bone you have, so it’s worth getting a proper assessment rather than ruling yourself out.

What Does The Process Actually Look Like?

Consultation and scan

Your dentist assesses your oral health and takes a 3D scan of your jaw to evaluate bone density and plan implant positioning.

Surgery.

The four implants are placed under local anaesthesia (or sedation, if preferred). At Dental On The Bay, this planning and placement can be supported by our X-Guide Dynamic 3D Navigation system for added precision.

Same-day teeth.

A full, fixed prosthesis is attached to the implants, so you leave with a functioning set of teeth rather than a gap.

Healing.

Over the following months, the implants fuse with your jawbone (osseointegration).

Final restoration.

Once healing is complete, your final, custom-fitted permanent bridge is placed, matched to your bite and desired appearance.

How Long Does All-on-4 Last?

With proper care that includes good oral hygiene, regular dental checkups, and avoiding habits like smoking or teeth grinding without a nightguard, All-on-4 implants can last well over a decade, and often for many years beyond that. Long-term studies on this implant approach report survival rates in the high 90-percent range.
